During the listening and discovering phase, a disability support worker takes the time to sit with you to discover the things you want to achieve; your goals, aspirations and dreams. We ask you about your work, education and recreational interests, along with many other questions. We really listen and make sure to discover what really matters to you.
Once we’ve asked the right questions, we write down the things you want to achieve and this is what we call your plan.
This plan becomes the basis for developing your tailor made supports that will lead to community inclusion. You will be involved in regularly discussing and reviewing your plan based on your experiences.
